CPSC, ZoloWear Announce Recall of Infant Carriers/Slings

Name of Product: ZoloWear Infant Carriers/Slings

Units: About 177

Manufacturer: ZoloWear Inc., of Austin, Texas

Hazard: The stitching that attaches the webbing to the carrier/sling can break, posing a fall hazard to young children.

Incidents/Injuries: ZoloWear has received one report of the webbing coming apart from the sling, but the baby was not in the sling at the time. The company has not received any reports of falls or injuries.

Description: The recalled slings are made of 100 percent cotton fabric or 97 percent cotton/3 percent Lycra with two pieces of webbing holding the rings to the fabric. Solid natural color and five prints (Splash, Pink and Black Stripe, Pink and Brown Stripe, Pink Punch and The Hamptons) make up the lots included in the recall. A large white label sewn on the pocket of the slings reads "Zolo." ZoloWear slings should have three rows of stitching securing the webbing and fabric together. Some of the slings in these lots have short webbing, so only one row of stitching holds the webbing in place.

Sold at: The ZoloWear.com Web site, individual distributors, and five children's boutiques in California, Hawaii and Texas sold these slings from May 2005 through August 2005 for between $70 and $90.

Manufactured in: U.S.

Remedy: Consumers should stop using these carriers/slings immediately and call ZoloWear for instructions on having the carriers/slings repaired.

CPSC, Sycamore Kids Inc. Announce Recall of Mountain Buggy Jogging Strollers

Name of Product: Mountain Buggy Urban Single and Urban Double Jogging Strollers

Units: About 2,200

Distributor: Sycamore Kids Inc., of Fort Collins, Colo.

Hazard: The handlebar can crack or break causing the handlebar to possibly detach while in use, posing a risk of injury to young children.

Incidents/Injuries: Sycamore has received 10 reports of a handlebar either cracking or breaking when the stroller was pulled up or taken down stairs. No injuries were reported.

Description: The recalled strollers have a metal frame and a cloth seat with a sun canopy. The strollers were sold in a variety of solid colors, including navy, red, black, silver and orange. A metal plate above the footrest shows the Mountain Buggy logo with "Mountain Buggy" written underneath. "URBAN" also is written on the metal plates. The recalled Mountain Buggy Urban Single strollers have item number U1204-002, and serial numbers between 000000 and 0003342. The recalled Mountain Buggy Urban Double strollers have item number U2204-002, and serial numbers between 000000 and 006957. The serial number and model number are on the back of the metal plate.

Sold by: Baby furniture and baby product stores nationwide and Web retailers from December 2004 through July 2005 for between $400 and $600.

CPSC, Dorel Juvenile Group Issue Alert on Safety 1st Tubside Bath Seats

In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Dorel Juvenile Group, of Columbus, Ind., announced a warning today for parents and caregivers who purchased the Safety 1st Tubside Bath Seats. The company has sold about 250,000 units to date. The bath seats are not intended and should not be used with certain non-traditional or sunken bathtubs. In these types of tubs, the bath seat can break, tip over and a child can fall into the water.

Dorel Juvenile Group has received nine reports of breakage due to use of Tubside Bath Seats in non-traditional or sunken bathtubs. An additional 67 reports of breakage due to handling, assembly and unknown reasons also were received. There was one report of a child bumping his forehead when a bath seat tipped over in a non-traditional tub, resulting in a bruise.

These Safety 1st Tubside Bath Seats have model number 44301 engraved on the bottom of the base. "Safety 1st" is printed on the front of the units. They are white and blue, have a swivel seat and an elbow cushion on the attachment arm. These seats are intended for use with children from 5 months to 10 months of age.

Consumers are urged to ensure that these bath seats are used in accordance with the instructions:

  • This product is NOT for use with the following style tubs:
  • Tubs with edge width less than 3 1/2 inches or greater than 5 1/2 inches.
  • Tubs with depth (top to bottom) less than 12 inches or greater than 15 inches.
  • Free standing (legged or pedestal), oval, drop-in or spa type tubs
  • DO NOT use the bath seat if the Red Warning Label on the arm is exposed.
  • ALWAYS keep child within arm's reach. Children have drowned when left unattended in bath seats.

Discount department and other mass merchants sold these bath seats from December 2003 through August 2004 for about $20.

Consumers who have a Tubside Bath Seat should check the dimension of their tubs. If they have a non-traditional or sunken bathtub outside the specified dimensions, or have a broken unit, the consumer should immediately stop using the Tubside Bath Seat and contact the firm for information on how to get a $20 rebate toward the purchase of any other Dorel Juvenile Group product. Consumers should not return product to retail stores.

CPSC, Graco Children's Products Inc. Announce Recall of Duo Tandem and Certain MetroLite Strollers

The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of strollers sold between 1994 and 2002. Consumers should stop using recalled products immediately until a repair kit has been obtained.

Name of Products: Graco Duo Tandem Stroller, Graco MetroLite Stroller

Units: About one million Duo Tandem and about 143,000 MetroLite strollers

Manufacturer: Graco Children's Products Inc., of Exton, Pa.

Hazard: These strollers can fail to latch properly and unexpectedly collapse while in use. This can result in broken bones, cuts, bumps, bruises and other injuries to young children riding in the stroller and consumers pushing the stroller.

Incidents/Injuries: For the Duo Tandem strollers, Graco has received reports of 306 collapses causing 230 reported injuries, including a broken arm, and a cut to a child requiring 46 stitches.

For the MetroLite strollers, Graco has received reports of 223 stroller collapses causing 34 reported injuries including 18 bumps and bruises to the head or body. Other injuries associated with both strollers include cuts, scrapes, scratches, pinched fingers and muscle pulls.

Description: The recalled Duo Tandem strollers, manufactured between 1994 and 1999, have two seats, one seat in front and one seat in back. The strollers have a blue, white or green plastic and steel frame. They have four wheels in the front and two wheels in the rear. The cloth seats and tops have various colors and patterns. There is a label with the model and serial number on the stroller's frame. Only strollers with serial numbers and model numbers listed are included in this recall.

The recalled MetroLite strollers, manufactured in 2000 and 2001, have a blue, gray or black plastic and steel frame. The cloth seat and top have various colors and patterns. The strollers have four wheels in the front and two wheels in the rear. These strollers were sold as a stand-alone stroller and also as part of a travel system that included an infant car seat/carrier and a base. The car seat/carrier is not affected. There is a label on the cross bar under the footrest containing the model and serial number. Only strollers with serial numbers between 10012000 and 12312001 in the first 8 digits and the following model numbers are included in this recall.

Remedy: Consumers should stop using these strollers immediately and contact the firm to receive a free repair kit. The kit includes a custom-designed latch that consumers should attach to the stroller's frame to ensure it is properly latched. The repair kit will be available in approximately one to two weeks.

CPSC, Prestige Toy Corp. Announce Recall of Spinning Water Teethers

Name of Product: Spinning Water Teethers

Units: About 1,500

Manufacturer: Prestige Toy Corp., of New York, N.Y.

Hazard: The plastic spinner in the center of the teether can break, causing small beads to fall out. This poses a choking hazard to young children.

Incidents/Injuries: Prestige Toy Corp. received one report from a consumer that the spinning component of one of these teethers broke apart, releasing two small beads. No injuries were reported.

Description: The water-filled teether is bright green or blue with a green and blue plastic butterfly or turtle in the center, which spins around. The spinner contains small beads, which produce a rattling sound. They have Style #47447 (butterfly) and #47448 (turtle) written on the packaging.

Sold At: Carter's Stores nationwide from March 2005 through April 2005 for about $5.

Remedy: Consumers should take these teethers away from young children and return the item to Prestige Toy Corp. for a full refund.

Del Pharmaceuticals Recalls Orajel Toddler Yellow Manual Toothbrush

Hazard: Del Pharmaceuticals, Inc. is initiating a voluntary recall of all lots of the Orajel Toddler Yellow Manual Toothbrush. This decision has been made following receipt of five reports of elastomeric heads of these toothbrushes breaking during use. A broken toothbrush head could pose a choking hazard to a young child.

Incidents/Injuries: Although the toothbrush was extensively tested prior to launch and found to meet established child use and abuse testing guidelines, it has been determined that the head can break under certain use conditions. The brushes are being recalled to eliminate this risk. Although the company has received no reports of injuries, in the interest of ensuring child safety, Del has decided to voluntarily recall the toothbrush.

Description: Del Pharmaceuticals is committed to the quality and safety of all the products that it sells. This decision, to discontinue the Toddler Manual Toothbrush and issue this voluntary recall, is in accordance with that commitment. Only the Orajel Toddler Yellow Manual Toothbrush is affected by this announcement. Both the Baby Orajel Manual Toothbrush and the Orajel Toddler Battery Toothbrush are unaffected and should continue to be used.

Sold at: The product has been sold at retail drugstores and supermarkets in the United States since August 2004.

The toothbrush is yellow, with yellow rubber-like bristles. There is a picture of a dancing bear on the handle.

Remedy: Consumers who have the Toddler Yellow Manual Toothbrush should stop use and return it to the company for a full refund.

CPSC, Delta Enterprise Corp. Announce Recall of Lov's Pacifiers

Name of Product: Lov's Decorated Orthodontic Pacifier

Hazard: The pacifiers are banned under federal law. They failed federal safety tests when the nipples separated from the base. This poses a choking hazard to young children.

Incidents/Injuries: CPSC has received one report of a 6-month-old child who was found gagging on the nipple that had separated from the pacifier. The child was not injured.

Description: The recalled pacifier measures about 2 1/4 inches in width and 1 1/2 inches in length and has button or hinged handles. The pacifiers are white with various colored handles. Designs are imprinted on some pacifier shields. One pacifier shield is decorated with a pink elephant, a purple giraffe with green polka dots, a blue lion with yellow fur and a blue cloud and yellow sun. Another pacifier shield has blue stars with Santa shapes. The amber colored nipple, imprinted with the words "caoui," "chouc" and "pur," is about 1 1/8 inches long. "Lov 2-Pack," "Decorated Orthodontic Pacifier" and "99705" are printed on the front packaging of the pacifiers. "Delta Enterprise Corp., Brooklyn NY 11212 Made in Thailand" is printed on the package back.

Sold at: Small retail stores from November 2001 through December 2004 for about $1.

CPSC, Simplicity Inc. Announces Recall of Cribs

Hazard: The white paint on the cribs can chip, posing a choking hazard to young children.

Incidents/Injuries: Simplicity Inc. has received four reports of consumers who noticed paint peeling off their crib. No injuries have been reported.

Description: The recalled cribs are made of wood and painted white. Model numbers 8554W-PT and 8554WW are printed inside the headboard and on the envelope attached to the mattress support. The Simplicity name, address, manufacturing date and model number are written on a label found on the inside bottom of the headboard.

Sold at: Department stores, Children's product stores and Target.com from June 2004 through April 2006 for about $200.

CPSC, Big Save International Announce Recall of Baby Walkers

Name of product: Baby Walker

Hazard: The walkers fit through a standard doorway and are not designed to stop at the edge of a step. Additionally, these walkers can tip-over. Babies using these walkers can be seriously injured or killed.

Incidents/Injuries: None.

Description: Only Big Save baby walkers with model numbers 9026, 9028, 9090, 90110 and 90111 are included in this recall. The model number is printed as the final digits on a bar code number and may appear on the front or back of the walker. Each walker has six caster wheels and comes in various colors: light green, pink and blue.

Sold at: Toy and juvenile product stores nationwide for between $15 and $25. Model 9026 walkers were sold from June 1998 through December 2002 and all other models were sold from December 2003 through December 2004.

CPSC, Orbelle Trade Inc. Announce Recall of Cribs

Name of product: Cribs

Manufacturer: Orbelle Trade Inc., of Brooklyn, N.Y.

Hazard: The Amber model crib included in this recall does not comply with crib safety standards due to a gap between the side rail and the crib mattress support, posing an entrapment hazard to infants. All other model cribs included in this recall do not have proper assembly instructions and diagrams required for cribs. They are also missing cautionary and warning labels as required by federal law. If the cribs are not assembled properly they could pose an entrapment hazard to infants.

Incidents/Injuries: None reported.

Description: The cribs included in this recall are the Daniella, Leeat, Noa, Naomi, Gabriella, Amber and Series 300 model cribs. The cribs are wood and come in a variety of painted and stained finishes including natural, cherry, blonde or white wood. Some of the cribs have built-in storage drawers on the side of the crib or underneath the crib. The crib model name may be printed on the lower part of the headboard of each crib and the manufacturer name is printed on each set of crib warnings when provided.

Sold at: Juvenile furniture stores in New York and New Jersey from January 2003 through August 2004 for between $135 and $335.

Remedy: If you own the Amber model crib, contact the firm to schedule an in-home repair or request a replacement crib. For all other model cribs, contact the firm to receive new assembly instructions and warning labels.

Incidents/Injuries: CPSC has received one report of a pacifier nipple that detached from the pacifier while in the mouth of a 7-month-old. The child was not injured.

Description: The pacifiers, which measure 3 inches long and about 1 1/2 inches across the base, are pink, blue, green or white plastic with a yellow nipple. The pacifiers have a circular handle and the shield is labeled "Jaloma." The nipples are filled with a sugary substance.

Sold at: Various small Mexican supermarkets and commercial retailers nationwide from March 2004 through August 2004 for between $0.35 and $1.

Manufactured in: Mexico

Remedy: Consumers should immediately take the pacifiers away from young children and return them to the store where purchased for a full refund.

CPSC, PlayKids USA Announce Recall of Baby Walkers

Hazard: The walkers will fit through a standard doorway and are not designed to stop at the edge of a step. Babies using these walkers can be seriously injured or killed if they fall down stairs.

Description: These baby walkers were sold under the brand name "Playkids USA." They have model numbers PLK94STP, PLT95STP, PLK2000RC, PLK95, PLK94, PLK98STP, PLK2000 or PLK300 on the package and on a sewn-in label on the seat back. The plastic walkers have eight snap-in wheels, an activity tray with toy wheels, knobs and lights, and a padded fabric seat. They come in a variety of colors and seat fabrics. The sewn-in seat label also reads "PLAYKIDS U.S.A. BROOKLYN, NY 11229 MADE IN TAIWAN."

Sold at: Small independent specialty juvenile retailers nationwide sold the baby walker from February 2003 through April 2004 for between $29 and $39.

Remedy: Consumers should stop using the recalled walkers immediately and contact PlayKids to receive a full refund.

CPSC, Regal Lager Announce Recall to Repair "Baby Björn" Infant Carriers

Hazard: The back support buckle can detach from the shoulder straps, posing a fall hazard to the baby.

Incidents/Injuries: The firm has received 93 reports of the back support buckle detaching from the shoulder straps on the infant carrier. No injuries have been reported.

Description: The recalled infant carriers were sold under brand name "Baby Björn," which is printed on the front of the carriers and on the black molded plastic back support buckle. Model number 1-260 is printed on the care label on the strap of the carrier. The carriers are made of a cotton polyester blend fabric in black with red piping and blue with white piping. "Baby Carrier Active" is written on the packaging and above the warning label on the carrier.

Sold at: Specialty retail stores, catalogs and Internet sites from September 2003 through August 15, 2004, for about $120.

Remedy: Consumers should stop using the carrier immediately and contact Regal Lager for instructions on returning the carriers for repair.

Description: The pillows have a blue or peach-colored print fabric showing yellow moons, hearts and stars, or tan and blue teddy bears. The log-shaped pillows measure about 5 feet in length. "THERALINE," "THE BIG V" and "Finest Micropearl Filling" are written on a fabric tag sewn on the pillow.

Sold at: Independent specialty stores sold the pillows from June 2003 through May 2004 for about $60.

Description: The "C"-shaped pillows are about 36 inches long, 10 inches wide and 5 inches deep. There is a 100 percent cotton hypoallergenic cover of various colors and prints over the white pillow. The outer cover is removable. The whole pillow is washable. The fabric tag on the pillow reads, "Boston Billows, Inc."

Sold at: Hospitals and independent specialty stores from February 2000 through December 2003 for between $34 and $38.

CPSC, Regal Lager Inc. Announce Recall to Repair Stroller Attachments

Name of product: Lascal "Buggy Board" stroller attachment

Hazard: The Buggy Board's red connecting pins can break, causing the board to partially detach from the stroller or carriage, posing a fall hazard to the user.

Incidents/Injuries: Regal Lager Inc. has received two reports of the Buggy Board disengaging from a stroller or carriage, resulting in falls to children. One minor injury was reported.

Description: The Lascal "Buggy Board" is a wheeled board that attaches to a stroller or baby carriage, allowing an older child to stand on the board and ride behind the stroller between the arms of a parent or caregiver. The product comes in black, blue, red and techno styles, and has the words "Buggy Board" printed on the top center of the board, with the name "Lascal" molded into the plastic on the bottom center of the board. Buggy Boards with orange connecting pins are not included in the recall.

Sold at: Specialty retail stores, catalogs and internet sites from January 2003 through July 2004 for about $88.

Remedy: The manufacturer is providing free repair kits to consumers consisting of orange replacement pins for these Buggy Boards.

CPSC, Graco Children's Products Announce Recall of Travel Lite Swings

Hazard: The swing's carrying handle can fail to stay in place properly and drop or be pushed down, hitting a child in the head. Additionally, the three-point seatbelt can fail to prevent a child from leaning forward or to either side, posing a risk that the child can fall forward and strike his/her head on the floor or the swing's frame.

Incidents/Injuries: Graco has received about 28 reports of incidents involving the handle falling down on young children; in addition, Graco has received 100 reports of children falling forward or to the side. Injuries resulting from these incidents include bloody or swollen lips, red marks, bumps and bruises.

Description: The recalled Travel Lite portable swings have an adjustable reclining seat, a rotating handle and a canopy and include model numbers 1850JJP, 1850JGB and 1870DAL. The swings, which were manufactured between May 2003 and December 11, 2003, also have a serial number between 050503 and 121103. Both the model and serial numbers can be found on a white label underneath the seat. The swings have the words, "Graco" and "Travel Lite Swing" printed on each side, and have buttons on the handle to activate lights and music. On the underside of the handle are multicolored designs of the sun, moon and stars that light up when the light button is pressed.

Sold at: Discount, department and juvenile stores from June 2003 through June 2004 for about $60.

Remedy: Consumers who have a Travel Lite Swing with a three-point seatbelt (waist belt and crotch strap only) or a Travel Lite Swing without a red handle release button should stop using it immediately and contact Graco for a free repair kit.
